The landscape of liver disease management has changed dramatically in recent years. Oral antivirals against hepatitis C virus (HCV) and hepatitis B virus (HBV) have been epochal breakthroughs, curing millions of infected people worldwide and reducing liver-related mortality. In addition, the first targeted therapy against hepatitis delta (HDV) has become available and its long-term clinical effects are being studied.
In this scenario, the clinical importance of other diseases is gradually increasing. The management of both metabolic-dysfunction associated steatotic liver disease (MASLD) and alcohol-related liver disease (ALD) remains challenging due to the lack of effective pharmacological options. On the other hand, new therapeutic approaches have become available to treat some rarer diseases, including primary biliary cholangitis (PBC).
Nevertheless, cirrhosis-related complications, such as hepatocellular carcinoma (HCC) and portal hypertension, remain a significant threat to patient outcomes in all etiologies. Liver transplantation remains an option for most patients and is expanding beyond traditional indications.
Improving patient survival and quality of life is essential and requires personalized management strategies that take advantage of multidisciplinary approaches and technological advances.
